I believe that they don't care that much about the "risk", they care about their story. Miracle Drug fits perfectly in the encore, between Stephen Hawking's speech and the HIV-themed Streets. That's why it's there, not because it sounds good or because people -either casuals or die-hards - would like it.
Troubles is the transition between blaming other people (the whole first half and the second up until Bono justifies himself to his younger self) to understanding that actually "the trouble is you"... that fits where they originally intended, but musically it falls flat in that moment... so it's not that easy to experiment with the spots (definitely not the closer, that's not how their story seems to end).
Or probably they're not telling any story and they're just being lazy and obtuse.

And anyway, a pre-show pic wouldn't mean anything as Dallas has to pick up that guitar (and some others), tune them and rehearse the amps and effects a good while before the show. He could or could not return that white Strat to its place after that! Nah, I wouldn't speculate based on this pic.
